AMENDMENT TO HOUSE BILL 4484

AMENDMENT NO. ______. Amend House Bill 4484 on page 1 by replacing line 12 with the following:

"(1) has previously failed to attend an appointment for which notice was provided at least 5 days before the appointment;

(2) has been notified of the time and place of the appointment at least 5 days before the independent"; and

on page 1, line 14, by changing "(2)" to "(3)"; and

on page 1, line 16, by changing "(3)" to "(4)".
