AMENDMENT TO HOUSE BILL 4128




2    AMENDMENT NO. ______. Amend House Bill 4128 on page 6, by 
3replacing lines 6 through 11 with the following:
 
4    "(225 ILCS 725/8d new)
5    Sec. 8d. False or inaccurate information. No person shall 
6provide false or inaccurate information on or relative to any 
7application, permit, required record, or other document 
8required to be submitted to the Department by this Act or any
