AMENDMENT TO HOUSE BILL 4781

AMENDMENT NO. ______. Amend House Bill 4781, AS AMENDED, with reference to page and line numbers of House Amendment No. 1, as follows:

on page 2, line 15, by replacing "licensed" with "licensed,"; and

on page 31, line 25, by replacing "fee." with "fee, except for a one-time enrollment fee of no more than $50."; and

on page 32, line 2, by replacing "10%" with "15%"; and

on page 34, line 2, immediately after "of", by inserting "the application fee and"; and

on page 42, line 23, by replacing "licensed" with "licensed,"; and

on page 42, line 24, by replacing "practice" with "practice,".
