| AMENDMENT TO HOUSE BILL 6299

| AMENDMENT NO. ______. Amend House Bill 6299 by replacing | | 3 |
| everything after the enacting clause with the following:

| "Section 5. The Illinois Insurance Code is amended by | | 5 |
| adding Section 356z.6 as follows:

| (215 ILCS 5/356z.6 new) | | 7 |
| Sec. 356z.6. Vision therapy. | | 8 |
| (a) An individual or group policy of accident and health | | 9 |
| insurance or managed care plan amended, delivered, issued, or | | 10 |
| renewed in this State after the effective date of this | | 11 |
| amendatory Act of the 93rd General Assembly, must include | | 12 |
| coverage for vision therapy, therapeutic lenses, prisms, | | 13 |
| filters, occlusions, and specialized equipment used in the | | 14 |
| vision therapy treatment process. | | 15 |
| (b) For the purposes of this Section, "vision therapy" | | 16 |
| means a treatment process used to improve vision function. | | 17 |
| Vision therapy includes a broad range of developmental and | | 18 |
| rehabilitative treatment programs individually prescribed to | | 19 |
| remediate specific sensory, motor, or visual perception | | 20 |
| dysfunctions.

| Section 10. The Health Maintenance Organization Act is | | 22 |
| amended by changing Section 4-6.5 as follows:
