AN ACT concerning health.

Be it enacted by the People of the State of Illinois, represented in the General Assembly:

Section 5. The Mental Health and Developmental Disabilities Code is amended by changing Sections 3-201 and 4-202 as follows:

(405 ILCS 5/3-201) (from Ch. 91 1/2, par. 3-201)

Sec. 3-201. The Department shall prescribe all forms necessary for proceedings under this Chapter, and all forms used in such proceedings shall comply substantially with the forms so prescribed. The Department shall publish all forms in electronic format and post the forms to its website print and furnish an initial supply of such forms to the clerks of the circuit courts.
