1 HOUSE RESOLUTION
2 WHEREAS, The members of the Illinois House of
3 Representatives are pleased to recognize milestone events in
4 the lives of the citizens of the State of Illinois; and
5 WHEREAS, It has come to our attention that Frederick
6 Jaffe recently fulfilled the requirements for qualification
7 as a Fellow of the American Society of Consultant
8 Pharmacists; and
9 WHEREAS, The American Society of Consultant Pharmacists
10 Fellowship is a special honor bestowed upon pharmacists who
11 meet the highest standards in Senior Care Pharmacy and have
12 demonstrated an extraordinary level of service in
13 professional practice activities; and
14 WHEREAS, A Fellow of the Society must fulfill rigorous
15 criteria established by the American Society of Consultant
16 Pharmacists Board of Directors; a Fellow must demonstrate
17 dedication and achievement in professional activities,
18 educational activities, professional innovation, advocacy and
19 civic activities; an American Society of Consultant
20 Pharmacists Fellow has made a commitment to go above and
21 beyond the traditional pharmacy practice by distinguishing
22 themselves through exemplary service and contributions to the
23 practice of senior care pharmacy and to society; therefore,
24 be it
25 RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
26 NINETY-SECOND GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that
27 we congratulate Frederick Jaffe on his recent induction as a
28 Fellow of the American Society of Consultant Pharmacists; and
29 be it further
30 RESOLVED, That a suitable copy of this resolution be
31 presented to Frederick Jaffe as an expression of our esteem.
