 1        AN ACT concerning education.

 2        Be  it  enacted  by  the People of the State of Illinois,
 3    represented in the General Assembly:

 4        Section  5.  The Disabled Persons Rehabilitation  Act  is
 5    amended by changing Section 13a as follows:

 6        (20 ILCS 2405/13a) (from Ch. 23, par. 3444a)
 7        Sec.  13a.   (a)  The Department shall be responsible for
 8    coordinating the establishment of local  Transition  Planning
 9    Committees.   Members  of  the  committees  shall  consist of
10    representatives  from  special  education;   vocational   and
11    regular education; post-secondary education; parents of youth
12    with  disabilities; persons with disabilities; local business
13    or industry; the Department of  Human  Services;  public  and
14    private adult service providers; case coordination; and other
15    consumer,  school,  and  adult  services as appropriate.  The
16    Committee shall  elect  a  chair  and  shall  meet  at  least
17    quarterly.  Each Transition Planning Committee shall:
18             (1)  identify current transition services, programs,
19        and  funding  sources  provided  within the community for
20        secondary and post-secondary aged youth with disabilities
21        and  their  families  as  well  as  the  development   of
22        strategies to address unmet needs;
23             (2)  facilitate   the   development   of  transition
24        interagency  teams  to   address   present   and   future
25        transition   needs   of   individual  students  on  their
26        individual education plans;
27             (3)  develop a mission statement that emphasizes the
28        goals of integration and participation in all aspects  of
29        community life for persons with disabilities;
30             (4)  provide for the exchange of information such as
31        appropriate    data,   effectiveness   studies,   special

 1        projects, exemplary programs,  and  creative  funding  of
 2        programs;
 3             (5)  develop   consumer   in-service  and  awareness
 4        training programs in the local community; and
 5             (6)  assist  in  staff   training   for   individual
 6        transition   planning   and   student   transition  needs
 7        assessment.
 8        (b)  Each Transition Planning Committee  shall  select  a
 9    chair  from  among  its members who shall serve for a term of
10    one year.  Each committee shall meet at least  quarterly,  or
11    at such other times at the call of the chair.
12        (c)  Each  Transition  Planning  Committee shall annually
13    prepare and submit to the Interagency Coordinating Council  a
14    summary  which  assesses  the  level  of  currently available
15    services in the community as well as the level of unmet needs
16    of secondary students with disabilities.
17        (d) The name and affiliation  of  each  local  Transition
18    Planning  Committee  member  and  the annual summary required
19    under subsection (c) of this Section shall be filed with  the
20    administrative  office  of each school district served by the
21    local Transition Planning Committee and made available to the
22    public upon request.

24        Section  10.  The School  Code  is  amended  by  changing
25    Section 14-8.03 as follows:

26        (105 ILCS 5/14-8.03) (from Ch. 122, par. 14-8.03)
27        Sec. 14-8.03. Transition goals, supports, and services.
28        (a)  A  school  district shall consider, and develop when
29    needed,  the  transition  goals  and  supports  for  eligible
30    students with disabilities not later than by the school  year
31    in which the student reaches age 14 1/2 at the individualized
32    education  plan  program  meeting  and  provide  services  as

 1    identified  on  the  student's  individualized education plan
 2    program.   Transition   goals   shall   include   employment,
 3    post-secondary  education, and community living alternatives.
 4    Consideration  of   these   goals   shall   result   in   the
 5    clarification   of  a  school  district's  responsibility  to
 6    deliver specific  educational  services  such  as  vocational
 7    training and community living skills instruction.
 8        (b)  To  appropriately  assess and plan for the student's
 9    transition needs, additional  individualized  education  plan
10    team  program   members  may be necessary and may be asked by
11    the school  district  to  assist  in  the  planning  process.
12    Additional   individualized   education   plan  team  program
13    planning  members  may  include  a  representative  from  the
14    Department of Human Services, a case coordinator, or  persons
15    representing  other  community  agencies  or  services.   The
16    individualized  education  plan  program  shall  specify each
17    person who is responsible  for  coordinating  and  delivering
18    transition  services.  The public school's responsibility for
19    delivering educational services does not  extend  beyond  the
20    time  the  student  leaves school or when the student reaches
21    age 21.
22        (c)  A school district shall submit annually a summary of
23    each eligible student's transition goals and needed  supports
24    resulting  from  the  multidisciplinary  staff conference and
25    individualized education plan team  program  meeting  to  the
26    appropriate local Transition Planning Committee.  If students
27    with  disabilities  who  are ineligible for special education
28    services request transition  services,  local  public  school
29    districts   shall   assist   those  students  by  identifying
30    post-secondary school goals, delivering appropriate education
31    services, and coordinating with other agencies  and  services
32    for assistance.

 1        Section  99.  Effective date.  This Act takes effect upon
 2    becoming law.
