Provides that the Department of Commerce and Community Affairs, the Department of Human Services, and the Department of Public Aid shall notify telecommunications carriers of the name and address of persons in the carriers' local exchange service area who are eligible for the Program. Requires carriers to provide service to eligible customers who fail to decline the service.
