Changes the short title to the School and Health Care Provider Visitation Rights Act. Requires an employer to grant an employee leave of up to 8 hours during a school year to accompany the employee's child to visits with a health care provider. Provides that no more than 4 of those hours may be taken in a single day. Provides that a health care provider shall give documentation of the visit to the employee and that the employee shall give the documentation to the employer.
