 1        AN ACT to amend the Unified Code of Corrections by adding
 2    Section 3-12-15.

 3        Be  it  enacted  by  the People of the State of Illinois,
 4    represented in the General Assembly:

 5        Section 5.  The Unified Code of Corrections is amended by
 6    adding Section 3-12-15 as follows:

 7        (730 ILCS 5/3-12-15 new)
 8        Sec. 3-12-15.  Personally identifiable information.
 9        (a)   For   purposes   of   this   Section,   "personally
10    identifiable information" includes, without  limitation,  the
11    following with respect to any individual:
12             (1)  Address.
13             (2)  Telephone number.
14             (3)  Fax number.
15             (4)  E-mail address.
16             (5)  Driver's license number.
17             (6)  Social Security Number.
18             (7)  Credit card number.
19             (8)  Credit history and credit rating.
20             (9)  Insurance information.
21             (10)  Information on purchasing habits.
22             (11)  Automobile registration information.
23             (12)  Vehicle  identification  number of any vehicle
24        owned or leased by the individual.
25        (b) The Department may not assign or permit any committed
26    person to enter any personally identifiable information in  a
27    computer  processible medium or any other medium, nor may any
28    other governmental entity or any private organization  assign
29    or permit any person committed to the Department to enter any
30    personally identifiable information in a computer processible
31    medium  or  any  other  medium  pursuant  to  an  interagency

 1    agreement or contract with the Department.
 2        (c)  This   Section   applies   to   all   contracts  and
 3    interagency agreements entered into by the Department  before
 4    the  effective  date of this amendatory Act of 1999 and still
 5    in existence on that date as well as  to  all  contracts  and
 6    interagency  agreements  entered into by the Department on or
 7    after that date.
