        Amends  the  Workers'   Compensation   Act   and   the   Workers'      
   Occupational  Diseases  Act.  Creates the State Compensation Insurance      
   Fund as an independent public corporation to insure employers  against      
   liabilities  for  certain injuries and occupational diseases for which      
   their employees may be entitled to benefits.  Provides  full  coverage      
   under  the  Workers'  Compensation Act for Chicago firefighters.  Adds      
   provisions regarding:  limits on charges  by  health  care  providers;      
   resolution   of  disputes  concerning  those  charges;  disclosure  of      
   provider self-referral; limits on collection efforts by providers; and      
   fees for medical records.   Makes  numerous  changes  in  relation  to      
   compensation  levels,  limitations  on  claims,  liability of parties,      
   penalties for failure to comply with the Acts,  presumptions  relating      
   to certain injuries and diseases, obligations of workers' compensation      
   insurers,  and  other matters.  Amends the Business Corporation Act to      
   require corporations to submit proof of workers' compensation coverage      
   to the Secretary of State.
