1 AMENDMENT TO HOUSE BILL 562
2 AMENDMENT NO. . Amend House Bill 562 on page 1, line
3 19, after the period, by inserting "This Section does not
4 apply to persons who are hard of hearing. However, it must
5 be certified by a physician licensed to practice medicine in
6 Illinois that the person is hard of hearing, and the
7 certification must be carried in the motor vehicle at all
8 times. The certification shall be legible and shall contain
9 the date of issuance, the name, address, and signature of the
10 attending physician, and the name, address, and medical
11 condition of the person requiring the exemption. The
12 information on the certification must remain current and
13 shall be renewed annually by the attending physician.".
