1    AN ACT concerning revenue.
 
2    Be it enacted by the People of the State of Illinois,
3represented in the General Assembly:
 
4    Section 5. The State Finance Act is amended by adding
5Sections 5.878 and 6z-102 as follows:
 
6    (30 ILCS 105/5.878 new)
7    Sec. 5.878. The Thriving Youth Income Tax Checkoff Fund.
 
8    (30 ILCS 105/6z-102 new)
9    Sec. 6z-102. Thriving Youth Income Tax Checkoff Fund;
10creation. The Thriving Youth Income Tax Checkoff Fund is
11created as a special fund in the State treasury. Moneys in the
12Fund shall be used by the Department of Human Services for the
13purpose of making grants to providers delivering non-Medicaid
14services for community-based youth programs in the State.
 
15    Section 10. The Illinois Income Tax Act is amended by
16adding Sections 507GGG and 507HHH as follows:
 
17    (35 ILCS 5/507GGG new)
18    Sec. 507GGG. Thriving Youth checkoff. For taxable years
19ending on or after December 31, 2017, the Department must print
20on its standard individual income tax form a provision (i)

1indicating that if the taxpayer wishes to contribute to the
2Thriving Youth Income Tax Checkoff Fund, as authorized by this
3amendatory Act of the 100th General Assembly, he or she may do
4so by stating the amount of the contribution (not less than $1)
5on the return and (ii) stating that the contribution will
6reduce the taxpayer's refund or increase the amount of payment
7to accompany the return. Failure to remit any amount of
8increased payment shall reduce the contribution accordingly.
9This Section does not apply to any amended return.
 
10    (35 ILCS 5/507HHH new)
11    Sec. 507HHH. Illinois Police Memorial checkoff.
12    (a) For taxable years ending on or after December 31, 2017,
13the Department must print on its standard individual income tax
14form a provision (i) indicating that if the taxpayer wishes to
15contribute to the Criminal Justice Information Projects Fund,
16as authorized by this amendatory Act of the 100th General
17Assembly, he or she may do so by stating the amount of the
18contribution (not less than $1) on the return and (ii) stating
19that the contribution will reduce the taxpayer's refund or
20increase the amount of payment to accompany the return. Failure
21to remit any amount of increased payment shall reduce the
22contribution accordingly. This Section does not apply to any
23amended return.
24    (b) Moneys deposited into the Criminal Justice Information
25Projects Fund under this Section shall be distributed equally,

1as soon as practical but at least on a monthly basis, to the
2Chicago Police Memorial Foundation Fund, the Police Memorial
3Committee Fund, and the Illinois State Police Memorial Park
4Fund. Moneys transferred to the funds shall be used, subject to
5appropriation, to fund grants for building and maintaining
6memorials and parks; holding annual memorial commemorations;
7giving scholarships to children of officers killed or
8catastrophically injured in the line of duty, or those
9interested in pursuing a career in law enforcement; and
10providing financial assistance to police officers and their
11families when a police officer is killed or injured in the line
12of duty.
 
13    Section 99. Effective date. This Act takes effect upon
14becoming law.
