AMENDMENT TO HOUSE BILL 3380

AMENDMENT NO. ______. Amend House Bill 3380, AS AMENDED, with reference to page and line numbers of House Amendment No. 1 as follows:

on page 2, line 18, after "guardian" by inserting "of the disabled person that is the subject of the request,"; and

on page 2, line 20, after "agent" by inserting "of the disabled person that is the subject of the request,"; and

on page 2, line 24, after "guardian" by inserting "of the minor that is the subject of the request,"; and

on page 2, line 26, after "parent" by inserting "of the minor that is the subject of the request"; and

on page 5, line 26, by replacing "behalf of a minor or disabled person" with "behalf of the minor or disabled person that is the subject of the security freeze"; and

on page 10, line 22, by replacing "parent or guardian" with "parent, guardian, or agent"; and

on page 11, line 2, after "(3) a" by inserting "durable".
