HOUSE RESOLUTION

 
2     WHEREAS, The members of the Illinois House of
3 Representatives are saddened to learn of the death of David M.
4 Hayes of Roseville, who passed away on August 23, 2007; and
 
5     WHEREAS, He was born on October 6, 1961 in Monmouth, the
6 son of Gary and Donna Smith Hayes; he married Karen Grant on
7 July 13, 1985 in Monmouth; and
 
8     WHEREAS, David Hayes attended the Roseville schools; he
9 worked as an Emergency Medical Technician for United Ambulance;
10 he worked for the Corman Memorial Homes; and he was a paramedic
11 for Galesburg Hospital Ambulance; and
 
12     WHEREAS, He was a member and past president of Roseville
13 Ambulance Service, a member of the Roseville Fire Department,
14 and District Fire Marshall for the R.S.P. & E. Fire Protection
15 District; and he was Assistant Chief of the Roseville Rescue
16 Squad; and
 
17     WHEREAS, David Hayes was a member and past Master of the
18 Roseville Masonic Lodge #519 and a member of the Scottish Rite
19 Bodies, Valley of Peoria; he was a Shriner for Mohammed Shrine
20 of Peoria, a member and president of the Roseville Township
21 Cemetery and of the Spark Plug Collectors; he was a member of

1 the Swan Creek United Methodist Church and a member of the
2 Warren County 911 board; and
 
3     WHEREAS, He was preceded in death by his maternal
4 grandparents and his paternal grandfather; and
 
5     WHEREAS, The passing of David Hayes is deeply felt by his
6 wife, Karen; his parents, Gary and Donna Hayes; his brother,
7 Jeff Hayes; and his paternal grandmother, Blanche Hayes;
8 therefore, be it
 
9     RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
10 NINETY-FIFTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we
11 mourn, along with his family and friends, the passing of David
12 M. Hayes; and be it further
 
13     RESOLVED, That a suitable copy of this resolution be
14 presented to the family of David M. Hayes as a symbol of our
15 sincere sympathy.
