| HOUSE RESOLUTION

| WHEREAS, The members of the House of Representatives of the | 3 |
| State of Illinois learned with regret of the death of Dr. | 4 |
| Warren Tuttle of Harrisburg on Friday, July 22, 2005; and
| 5 |
| WHEREAS, He was born on May 22, 1919, in Harrisburg to O.P. | 6 |
| Tuttle and Loudene Nyberg Tuttle; he married Jane Lasersohn | 7 |
| Seright Tuttle in April of 1983; and
| 8 |
| WHEREAS, Dr. Tuttle graduated from Harrisburg Township | 9 |
| High School in 1937 and attended the University of Illinois | 10 |
| pre-med program; he graduated Summa Cum Laude from the | 11 |
| University of Illinois Medical School in 1942 and completed his | 12 |
| medical and surgical internship at Presbyterian Hospital in | 13 |
| Chicago; and
| 14 |
| WHEREAS, Dr. Tuttle served his country during World War II, | 15 |
| achieving the rank of Captain, U.S. Medical Corps, Surgical | 16 |
| Services at Bushnell General Hospital, a 2,000 bed facility in | 17 |
| Brigham City, Utah; after the war, Dr. Tuttle returned to his | 18 |
| hometown of Harrisburg and faithfully served in the private | 19 |
| practice of medicine and surgery from 1945 to 2005, a 60-year | 20 |
| career; during that time, Dr. Tuttle and his partner, Dr. S.H. | 21 |
| Frazer, owned Lightner Hospital in Harrisburg from 1953 to | 22 |
| 1956; and
| 23 |
| WHEREAS, During the time that he was co-owner of Lightner | 24 |
| Hospital, the residents of the community experienced a great | 25 |
| improvement in the quality of care both in the area of medicine | 26 |
| and surgery; Dr. Tuttle was a major stockholder and executive | 27 |
| director of Doctors Hospital in Harrisburg from 1965 to 1975; | 28 |
| Doctors Hospital was reorganized in 1975 as a not-for-profit | 29 |
| corporation, and he continued to serve as executive director | 30 |
| until 1984; he remained on the Board of Directors of Harrisburg | 31 |
| Medical center from its inception until his recent passing, an |

| institution he dearly loved and dedicated and devoted his life | 2 |
| to; and
| 3 |
| WHEREAS, Dr. Tuttle had served as medical director of | 4 |
| quality assurance since 1990 and Harrisburg Medical Center was | 5 |
| recently awarded national praise for quality assurance under | 6 |
| his direction; he served his local, State, and national | 7 |
| communities by giving of himself and his expertise as a member | 8 |
| of the Saline, Pope, and Hardin County Medical Society (past | 9 |
| president); he was a member of the Southern Illinois Medical | 10 |
| Society (member of the board of directors), the Illinois State | 11 |
| Medical Society (member of the board of trustees), and the AMA; | 12 |
| he was also a member of the Fellow American College of | 13 |
| Gastroenterology and the Fellow American Academy of General | 14 |
| Practice; he was a past member of the American Society of | 15 |
| Abdominal Surgeons, a past member of the International College | 16 |
| of Surgeons, and member of the American Academy of Medical | 17 |
| Acupuncture; and | 18 |
| WHEREAS, He served as member of the Harrisburg High School | 19 |
| Board for 13 years, as a member of the board of trustees of | 20 |
| Southeastern Illinois College for six years, and was first | 21 |
| vice-president of Saline Valley First Federal Savings and Loan | 22 |
| in Harrisburg; he received the Citizen of the Year Award from | 23 |
| the Harrisburg Lions Club in 1970, and the University of | 24 |
| Illinois Alumnus of the Year Award for services in medical | 25 |
| organization in 1974; and | 26 |
| WHEREAS, Dr. Tuttle was a member of the Methodist Church | 27 |
| and served on numerous committees; he was also a member of the | 28 |
| T.B. Advisory Illinois Department of Public Health in 1971, a | 29 |
| member of the Illinois State Medical Society Board of Trustees | 30 |
| for 14 years, and the chairman of the Board of Trustees for the | 31 |
| ISMS from 1979 to 1982; he received the Distinguished Service | 32 |
| Award from the Illinois Lung Association in 1983; he was | 33 |
| chairman of the Saline County Tuberculosis Sanitary Board for |

| 39 years, past vice president of the Illinois Tuberculosis and | 2 |
| Respiratory Disease Association, a past member of the AMA | 3 |
| Council of Scientific Assembly, a past member of the Illinois | 4 |
| State Medical Examining Board for eight years, chairman of the | 5 |
| Board of Illinois State Medical Insurances Inc. from 1984 to | 6 |
| 1986, and a member of the Elks Club, the American Legion, and | 7 |
| Voiture 794; and | 8 |
| WHEREAS, The passing of Dr. Warren Tuttle has been deeply | 9 |
| felt by many, especially his wife, Jane; his children, Mrs. | 10 |
| Susie Tuttle Vogler and husband Frank and Mrs. Kim Tuttle | 11 |
| DiChiaro and husband Jerry; his stepdaughter, Mrs. Pam Seright | 12 |
| Pearson and husband Joe; his grandchildren, Gerard DiChiaro and | 13 |
| Francesca DiChiaro; his brother, Burdette (Bert); his nieces | 14 |
| and nephews, Warren and Pat Tuttle and Jack and Beth Tuttle and | 15 |
| her daughter, Julia Mae; and his cousin, Anna Gregory; he was | 16 |
| preceded in death by his son, Warren D. Tuttle (Buddy), and his | 17 |
| brother, Dr. John Tuttle; therefore, be it
| 18 |
| RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE | 19 |
| NINETY-FOURTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that | 20 |
| we mourn the passing of Dr. Warren Tuttle, and we extend our | 21 |
| sincerest condolences to his family, friends, and all who knew | 22 |
| and loved him; and be it further
| 23 |
| RESOLVED, That a suitable copy of this resolution be | 24 |
| presented to his family as an expression of our deepest | 25 |
| sympathy and in remembrance of the life that he devoted to the | 26 |
| medical care of his fellow citizens.
