 1        AN ACT concerning elections.

 2        Be  it  enacted  by  the People of the State of Illinois,
 3    represented in the General Assembly:

 4        Section  5.  The  Election  Code  is  amended  by  adding
 5    Section 28-3.1 as follows:

 6        (10 ILCS 5/28-3.1 new)
 7        Sec. 28-3.1.  For each provision of  law  in  this  State
 8    that  authorizes  registered voters to file petitions for the
 9    submission of a public question to voters  in  a  referendum,
10    the  State  Board  of  Elections shall prepare petition forms
11    with appropriate blank spaces that may be filled in by voters
12    who seek to circulate  and  file  the  petitions.   When  the
13    provisions  of  State  law  authorizing  the  filing  of  the
14    petitions  set  forth  specific  language  to  be used in the
15    petition, the  petition  forms  shall  contain  the  specific
16    language.
17        The   State  Board  of  Elections  shall  distribute  the
18    petition forms to each election authority and each  municipal
19    clerk  where  the petitions may be filed, for distribution to
20    voters who request them.  The State Board shall also keep  an
21    entire  set  of  the  petition  forms  at its Springfield and
22    Chicago offices for distribution to voters who request them.

23        Section 99.  Effective date.  This Act takes effect  upon
24    becoming law.
