| 4 | | 1, on page 2, by replacing lines 10 through 17 with the |

| 6 | | "(3.5) the parent or guardian of a minor, regardless |
| 7 | | of the minor's age, who is involved in special education |
| 8 | | services under Section 14-1.11 of the School Code, if the |
| 9 | | parent or guardian consented to mental health or |
| 10 | | developmental services on behalf of the recipient; or the |
| 11 | | designated representative of a student over the age of 18 |
| 12 | | involved in special education services under Section |
| 13 | | 14-6.10 of the School Code the personal representative |
| 14 | | under HIPAA, 45 CFR 164.502(g), of a recipient regardless |
| 15 | | of the age of the recipient;". |
