AMENDMENT TO HOUSE BILL 5459

2     AMENDMENT NO. ______. Amend House Bill 5459 on page 1, by
3 replacing lines 13 through 23 with the following:
 
4 "information is completely voluntary, that registration with
5 the Illinois Adoption Registry and Medical Information
6 Exchange is voluntary, that the person will remain anonymous if
7 he or she completes a Denial of Information Exchange, and that
8 the person has the option to provide medical information only
9 and still remain anonymous. The information packet must include
10 all of the following:
11         (1) (Blank) All Illinois Adoption Registry and Medical
12     Information Exchange application forms, including the
13     Medical Information Exchange Questionnaire and the web
14     site address and toll-free phone number of the Registry.";
15     and
 
16 on page 2, line 18, by inserting before the comma the

2 "of a parent"; and
 
3 by replacing lines 20 through 26 of page 2 and lines 1 through
4 13 of page 3 with the following:
5 "Medical Information Exchange to hospitals, police stations,
6 fire stations, and emergency medical facilities.
7     (b) The information packet given to a relinquishing parent
8 in accordance with this Act shall include, in addition to other
9 information required under this Act, the following:
10         (1) A brochure (with a self-mailer attached) that
11     describes this Act and the rights of birth parents,
12     including an optional section for the parent to complete
13     and mail to the Department of Children and Family Services,
14     that shall ask for basic anonymous background information
15     about the relinquished child. This brochure shall be
16     maintained by the Department on its website.
17         (2) A brochure that describes the Illinois Adoption
18     Registry, including a toll-free number and website
19     information. This brochure shall be maintained on the
20     Office of Vital Records website.
21         (3) A brochure describing postpartum health
22     information for the mother.
23     The information packet shall be designed in coordination
24 between the Office of Vital Records and the Department of
25 Children and Family Services, with the exception of the

1 resource list of providers of counseling services and adoption
2 agencies, which shall be provided by the hospital, fire
3 station, police station, sheriff's office, or emergency
4 medical facility.".
