    AMENDMENT NO. ______. Amend House Bill 824, AS AMENDED,

with reference to the page and line numbers of Senate Amendment 
4
No. 3, on page 1, in line 4,  by replacing "Section 5." with the 
5
following:

    "Section 5. The Election Code is amended  by adding Section 
7
9-7.10 as follows:

    (10 ILCS 5/9-7.10 new)

    Sec. 9-7.10. Business entities; registration; reporting; 
10
notice to contribution recipients.
11
    (a)  For the purposes of this Section, the terms "business 
12
entity",  "affiliated entity", and "affiliated person" have the 
13
meanings ascribed to those terms in Section 50-37 of the 
14
Illinois Procurement Code.
15
    (b)  Within 30 days after the date that a State contract is 
16
awarded to a business entity whose contracts with State

agencies, in the aggregate, annually total more than $50,000, 
2
that business entity  shall register with the State Board of 
3
Elections, stating that the business entity has such a contract 
4
or contracts.  The registration statement shall include the 
5
following:

    Name and address of the business entity 
7
    Name and address of the business entity's authorized 
8
representative or representatives 
9
    The identity of the affiliated entities and affiliated 
10
persons of the business entity 
 
11
VERIFICATION: 
12
    "I declare that this statement (including any accompanying 
13
schedules and statements) has been examined by me and to the 
14
best of my knowledge and belief is a true, correct, and 
15
complete statement as required by Article 9 of the Election 
16
Code. I understand that willfully filing a false or incomplete 
17
statement is a business offense subject to a fine of at least 
18
$1,001 and up to $5,000."

each business entity, affiliated entity, and affiliated

person.

    (c)  Each business entity required to register under 
4
subsection (b) shall report to the State Board the recipient, 
5
amount, and date  of each contribution made by the business 
6
entity or by an affiliated entity or an affiliated person of 
7
the business entity to a political committee subject to this 
8
Article. The reports required under this subsection shall be 
9
filed at the same times and for the same reporting periods as 
10
required for the filing by political committees of reports of 
11
campaign contributions under subsections (b) and (b-5) of 
12
Section 9-10 and of semi-annual reports of campaign 
13
contributions and expenditures under subsection (c) of Section 
14
9-10.

    (d) Each report of contributions under subsection (c) shall 
16
disclose:
17
        (1) the full name and mailing address of the business 
18
    entity, affiliated entity, or affiliated person who has 
19
    made one or more contributions or transfers of funds to a 
20
    political committee within the reporting period in an 
21
    aggregate amount or value in excess of $500;
22
        (2) the name and address of the political committee to 
23
    which each contribution was made;
24
        (3) the amount of each contribution or transfer made;
25
        (4) the date of each contribution or transfer; and
26
        (5) each loan to a political committee within the

    reporting period in an aggregate amount or value in excess 
2
    of $500, together with the full names and mailing addresses 
3
    of the lender and endorsers, if any, and the date and 
4
    amount of such loans.

    (e)  Any business entity required to register under 
6
subsection (b), and any affiliated entity or affiliated person 
7
whose identity is required to be disclosed under subsection 
8
(b), shall give notice to any political committee to which it 
9
makes a contribution, at the time of the contribution, of the 
10
fact that the relevant business entity has contracts with State 
11
agencies that, in the aggregate, annually total more than 
12
$50,000.
