| WHEREAS, The members of the House of Representatives of the |
3 |
| State of Illinois are pleased to congratulate William A. |
4 |
| Johnson, M.D., on being named Prevention Partnership's |
5 |
| Prevention Person of the Year; and
|
6 |
| WHEREAS, Dr. Johnson is the Medical Director at an internal |
7 |
| medicine private practice for Johnson Healthcare Providers, |
8 |
| Ltd.; he serves as the Medical Director of the HIV/AIDS clinic |
9 |
| at Luck Care Center; he is also medical director and a team |
10 |
| physician at the Chicagoland Central Vitas Hospice office, |
11 |
| medical director at Project Coat, and serves in numerous other |
12 |
| locations and capacities; and
|
13 |
| WHEREAS, He received a Bachelor of Science degree in |
14 |
| pharmacy from Purdue University, his Doctor of Medicine degree |
15 |
| from the University of Chicago Pritzker School of Medicine, and |
16 |
| completed his Residency in Internal Medicine at Rush |
17 |
| Presbyterian St. Lukes Medical Center; and |
18 |
| WHEREAS, He was honored in 2001 with the Monarch Award in |
19 |
| Medicine, in 2000 as Doctor of the Year at Roseland Community |
20 |
| Hospital, in 1986 with Honors for his Internal Medicine |
21 |
| Clerkship at the University of Chicago, Pritzker School of |
22 |
| Medicine, and in 1981, he became a member of Rho Chi Pharmacy |
23 |
| Honor Society; and
|
24 |
| WHEREAS, Dr. Johnson is president of the Board of Directors |
25 |
| of Southside Health Association and Johnson Healthcare |
26 |
| Providers, a member of the Board of Directors of Roseland |
27 |
| Community Hospital and Metropolitan Health Chicago |
28 |
| Association, and served on the Advocate Trinity Health Partners |
29 |
| Board of Directors from 1996 to 2000; he is member of numerous |
30 |
| professional and community organizations; therefore, be it
