AMENDMENT TO HOUSE BILL 5026

2    AMENDMENT NO. ______. Amend House Bill 5026 by replacing
3everything after the enacting clause with the following:
 
4    "Section 5. The Illinois Vehicle Code is amended by
5changing Section 3-643 as follows:
 
6    (625 ILCS 5/3-643)
7    Sec. 3-643. Mammogram license plates.
8    (a) The Secretary, upon receipt of an application made in
9the form prescribed by the Secretary, may issue special
10registration plates designated as Mammogram license plates.
11The special plates issued under this Section shall be affixed
12only to passenger vehicles of the first division and motor
13vehicles of the second division weighing not more than 8,000
14pounds. Plates issued under this Section shall expire
15according to the multi-year procedure established by Section
163-414.1 of this Code.

1    (b) The design and color of the plates is wholly within the
2discretion of the Secretary, except that the phrase
3"Mammograms Save Lives" following phrases shall be on the
4plates: (i) "Mammograms Save Lives" and (ii) "The Susan G.
5Komen Foundation". The Secretary may allow the plates to be
6issued as vanity plates or personalized under Section 3-405.1
7of the Code. The Secretary shall prescribe stickers or decals
8as provided under Section 3-412 of this Code.
9    (c) An applicant for the special plate shall be charged a
10$25 fee for original issuance in addition to the appropriate
11registration fee. Of this fee, $10 shall be deposited into the
12Mammogram Fund and $15 shall be deposited into the Secretary
13of State Special License Plate Fund, to be used by the
14Secretary to help defray the administrative processing costs.
15    For each registration renewal period, a $25 fee, in
16addition to the appropriate registration fee, shall be
17charged. Of this fee, $23 shall be deposited into the
18Mammogram Fund and $2 shall be deposited into the Secretary of
19State Special License Plate Fund.
20    (d) The Mammogram Fund is created as a special fund in the
21State treasury. All money in the Mammogram Fund shall be paid,
22subject to appropriation by the General Assembly and
23distribution by the Illinois Department of Public Health, to
24the Illinois Breast and Cervical Cancer Program for patient
25navigation services specifically for populations with the
26highest rates of breast cancer mortality in the State.
