SENATE RESOLUTION

 
2    WHEREAS, The members of the Illinois Senate are saddened to
3learn of the death of Specialist Henry Jarrett Mayfield Jr. of
4Hazel Crest, who passed away while on duty in Kenya, Africa on
5January 5, 2020; and
 
6    WHEREAS, SPC Mayfield Jr. was born to Henry Mayfield Sr.
7and Crystal Davis on October 14, 1996; he attended Lionel
8Hampton Elementary, William J. Bogan High School, and later
9graduated from Hillcrest High School in 2014; he then attended
10Northern Illinois University, where he majored in business; and
 
11    WHEREAS, SPC Mayfield Jr. began working as an Aircraft
12Fueler at Aircraft Service International Group (ASIG) in 2016;
13and
 
14    WHEREAS, SPC Mayfield Jr. enlisted in the U.S. Army on
15August 21, 2017; he attended Basic Combat Training at Fort
16Jackson, South Carolina and Advanced Individual Training at
17Fort Lee, Virginia; he was awarded the MOS of 91C Utilities
18Equipment Repairer; he was assigned to the 1st Battalion, 58th
19Aviation Regiment at Fort Rucker, Alabama; he was deployed with
20the 1st Battalion, 58th Aviation Regiment to Kenya, Africa in
21support of Operation Enduring Freedom-Horn of Africa in October
222019; his awards and decorations included the Combat Action

1Badge (posthumous), the Bronze Star Medal (posthumous), the
2Purple Heart (posthumous), the Army Achievement Medal, the
3National Defense Service Medal, the Global War on Terrorism
4Expeditionary Medal, the Global War on Terrorism Service Medal,
5the Overseas Ribbon, and the Army Service Ribbon; and
 
6    WHEREAS, SPC Mayfield Jr. committed to his faith at an
7early age and was baptized by Reverend Marion Johnson Sr. of
8Come Alive Ministry of Faith Church; he was affectionately
9known as "Boo-Boo" to his family and enjoyed spending quality
10time with his siblings and cousins; he is remembered for
11bringing joy to anyone who met him; and
 
12    WHEREAS, SPC Mayfield Jr. was preceded in death by his
13great grandmother, Jacqueline Overton; and his grandparents,
14Ronnie E. Mayfield Sr., Stephanie Mayfield, and Levola Reed;
15and
 
16    WHEREAS, SPC Mayfield Jr. is survived by his parents, Henry
17Mayfield Sr. and Carmoneta Horton-Mayfield; his biological
18mother, Crystal Davis; his fiancée, Ce ara Hughes; his
19grandmother, Annette Horton; his grandfather, Michael
20Thompson; his brothers, Ronald, Tyron, Michael, and Nicholas;
21his sisters, Danielle (Cornelia), Ciara, Taliyah, and
22Tyshauna; his uncles, Ronnie Jr. (Sonya), Ricky (Detrich),
23Anthony (Mattie), and Melvin; his aunts, Elizabeth, Chaquitta,

1Charron, Andrea, and Martina (Joseph); his niece and nephew;
2and his great aunts, uncles, and cousins; therefore, be it
 
3    RESOLVED, BY THE SENATE OF THE ONE HUNDRED FIRST GENERAL
4ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of
5Specialist Henry Jarrett Mayfield Jr. and extend our sincere
6condolences to his family, friends, and all who knew and loved
7him; and be it further
 
8    RESOLVED, That a suitable copy of this resolution be
9presented to the family of SPC Mayfield Jr. as an expression of
10our deepest sympathy.
