AMENDMENT TO HOUSE BILL 54

2    AMENDMENT NO. ______. Amend House Bill 54 on page 1, by
3replacing lines 11 through 14 with the following:
4"his or her appearance or a warrant of arrest. No less than 30
5days before the issuance of a warrant under this subsection, a
6notice shall be mailed to the offender by first class mail to
7the most recent address which the offender has provided to the
8court. The notice shall contain the following information:
9        (1) the amount of the fine which is due and owing;
10        (2) the docket number of the case in which the fine was
11    assessed;
12        (3) the due date for payment of the fine;
13        (4) instructions as to how payment of the fine may be
14    made;
15        (5) an explanation that willful refusal to pay the fine
16    may result in imprisonment; and
17        (6) instructions as to how the offender may request a
18    hearing to present evidence that the offender did not

1    willfully refuse to pay the fine, and that failure to pay
2    was the result of the offender's inability to pay the
3    fine.".
