Provides that whenever a patient is transferred from a hospital, nursing home, or assisted living facility the transferring hospital, nursing home, or assisted living facility, shall provide the receiving hospital, nursing home, or assisted living facility with a form that lists certain specified information about the patient. Provides that the Illinois Department of Public Health shall develop and publish the form that is to be used by the transferring hospital, nursing home, or assisted living facility.
