Creates the Illinois Religious Freedom Protection and Civil Unions Act. Contains findings. Contains provisions regarding religious freedom and construction. Provides that 2 persons may form a civil union if they: are not related by adoption or blood in any manner that would bar a civil union; are not in another civil union or marriage with any other living person; and are not under 18 years of age. Provides that: protections, benefits, and responsibilities of partners in a civil union are the same as those granted to spouses in a marriage; to the extent State laws adopt, refer to, or rely upon provisions of federal law as applicable to this State, partners in a civil union shall be treated under the law of this State as if federal law recognized a civil union in the same manner as the law of this State; the Civil Practice Law applies to proceedings under this Act, except as otherwise provided; and a civil union between 2 persons of either the same sex or the opposite sex licensed, officiated, and registered under the Act is valid in this State. Contains provisions regarding: civil union licenses, civil union certificates, medical information brochures, effectiveness of licenses, officiation of civil unions, registration of civil unions, reports, prohibition of certain civil unions, children born or adopted of prohibited civil unions, reciprocity, severability, and other matters. Amends the Illinois Marriage and Dissolution of Marriage Act and the Illinois Human Rights Act to add references to civil unions. Defines "civil union" as a legal relationship between 2 persons, of either the same or opposite sex, established in accordance with the Act. Provides that a party to a civil union shall be entitled to the same legal obligations, responsibilities, protections, and benefits afforded or recognized by the law of Illinois to spouses. Prohibits certain civil unions. Provides that the Director of Public Health shall prescribe forms for an application, license, and certificate for a civil union. Contains provisions regarding: application for a civil union license; certification of a civil union; and duties of the county clerk and Department of Public Health. Provides for dissolution and declaration of invalidity of a civil union. Provides that a marriage between persons of the same sex, a civil union, or a substantially similar legal relationship other than common law marriage, legally entered into in another jurisdiction, shall be recognized in Illinois as a civil union.
