<reference>215 ILCS 5/368d new</reference><aliasreference></aliasreference><SynopsisText>     Amends the Illinois Insurance Code. Requires that a health care professional or provider be given a complete copy of any contract for professional services and be allowed at least 30 days to review the contract before being required to sign the contract. Requires that notice of proposed changes in the contract be given at least 90 days before the change takes effect. Effective December 1, 2003.</SynopsisText><synopsistitle>House Amendment No. 1</synopsistitle>
<SynopsisText>Replaces the title and everything after the enacting clause. Amends the Illinois Insurance Code. Requires that a health care professional or provider be given a complete copy of any contract for professional services and be allowed at least 30 days to review the contract before being required to sign the contract. Requires that notice of changes in the contract be given within 35 days of the effective date of the change.
