Provides that a check-off box for donations to the Blindness Prevention Fund be printed on the applications for vehicle registration, vehicle registration renewal, driver's licenses, and driving instruction permits. Provides that contributions to the Blindness Prevention Fund be used to provide blindness prevention education, screening, and treatment.
