| 6 | | to the personal representative, if known, of the patient"; and

| 7 | | on page 2, by replacing lines 8 and 9 with the following: |
| 8 | | "representative, if known, of the recipient of services as |
| 9 | | specified in this Section. The facility shall attempt to |
| 10 | | provide verbal notice to the personal representative, if"; and
|
| 11 | | on page 2, line 12, by inserting ", if known," after |
| 12 | | "representative". |
