| 2 | | Be it enacted by the People of the State of Illinois, |
| 3 | | represented in the General Assembly:
|
| 4 | | Section 5. The Vital Records Act is amended by adding |
| 5 | | Section 25.2 as follows:
|
| 6 | | (410 ILCS 535/25.2 new) |
| 7 | | Sec. 25.2. Office of State Guardian birth record request. |
| 8 | | (a) For purposes of this Section, an individual's status |
| 9 | | as a person under guardianship with the Office of State |
| 10 | | Guardian may be verified with a copy of the court order placing |
| 11 | | the individual under the guardianship of the Office of State |
| 12 | | Guardian. |
| 13 | | (b) The applicable fees under Section 17 for a new |
| 14 | | certificate of birth and under Section 25 for a search for a |
| 15 | | birth record or certified copy of a birth record shall be |
| 16 | | waived for requests made by the Office of State Guardian to the |
| 17 | | Office of the State Registrar of Vital Records in Springfield |
| 18 | | for an individual under guardianship of the Office of State |
| 19 | | Guardian, whose status is verified under subsection (a) of |
| 20 | | this Section. |
| 21 | | (c) The State Registrar of Vital Records shall establish |
| 22 | | standards and procedures consistent with this Section for |
| 23 | | waiver of the applicable fees. |
