AMENDMENT TO SENATE BILL 512 




2    AMENDMENT NO. ______. Amend Senate Bill 512, AS AMENDED, 
3with reference to the page and line numbers of Senate Amendment 
4No. 1, as follows:


 
5on page 5, immediately below line 3, by inserting the 
6following:
7        "9. A statement indicating if the applicant, including 
8    any of the applicant's affiliated or predecessor 
9    corporations, has been subject to the revocation of a 
10    business license by a municipality under Section 5-501.5 of 
11    this Code."; and
 
12on page 10, line 9, by deleting "and"; and
 
13on page 10, immediately below line 9, by inserting the 
14following:
15        "(1.5) notify the Secretary of the revocation; and";

2on page 10, by replacing line 12 with "licensee's business 
3license has been revoked due to fraud or misconduct committed 
4against the municipality.".
