| 2 | | AMENDMENT NO. ______. Amend Senate Bill 3409 on page 1, |
| 3 | | lines 17 through 19, by replacing "with notification to the |
| 4 | | patient's physician and with appropriate reporting and record |
| 5 | | retention." with ". Vaccination of a patient by a dentist shall |
| 6 | | be documented in the patient's dental record and the record |
| 7 | | shall be retained in accordance with current dental |
| 8 | | recordkeeping standards. The dentist shall notify the |
| 9 | | patient's physician of each dose of vaccine administered to a |
| 10 | | patient and shall enter all patient level data or update the |
| 11 | | patient's current record in the immunization data registry |
| 12 | | maintained by the Department of Public Health. The dentist may |
| 13 | | provide this notice to the patient's physician electronically. |
| 14 | | The dentist shall also use the immunization data registry |
| 15 | | system to review vaccination history for a patient prior to the |
| 16 | | administration of any vaccine.".
