   105 ILCS 5/18-8.05                                                          

        Amends the Illinois Income Tax Act.  Provides that all individual      
   income tax return forms for taxable  years  2001  through  2010  shall      
   contain  an appropriate space in which the taxpayer shall identify the      
   name, birth date, and social security number of each dependent claimed      
   on the tax return.  Provides that  the  Department  of  Revenue  shall      
   provide  the  Illinois  State Board of Education, for the previous tax      
   year and by elementary and secondary school district, with the age and      
   social  security number of each dependent claimed by a taxpayer in any      
   family whose income is below the federal poverty  level.   Amends  the      
   School  Code.    Beginning with the 2002-2003 school year, adds to the      
   formula for  determining  "low-income  concentration  level"  (i)  the      
   annual  family  income  information  received  from  the Department of      
   Revenue and (ii) any other factors  considered  to  be  necessary  and      
   appropriate  in  the  education of at-risk children.  Provides for the      
   creation of a Committee on Supplemental Education for At-Risk Children      
   for development of a formula for consideration  in  the  State  fiscal      
   year 2002 budget.  Effective immediately.
