LEITCH. 410 ILCS 50/3 from Ch. 111 1/2, par. 5403 Amends the Medical Patient Rights Act. Provides for the right of each patient, upon the written request of the patient or the patient's guardian or legally authorized representative, to be furnished a copy of the record of the patient's health history and treatment rendered to the patient by a physician or other health care provider.
