HOLBROOK. 320 ILCS 25/5 from Ch. 67 1/2, par. 405 Amends the Senior Citizens and Disabled Persons Property Tax Relief and Pharmaceutical Assistance Act. Provides that upon request, the Department must provide to an individual allowed payments under this Act a form to designate another address to receive the payments instead of the individual's residence. Provides that the individual may rescind the address designation.
