| 2 | | WHEREAS, The members of the Illinois Senate are saddened to | 3 | | learn of the death of Donna Jeanne Moncivaiz of Beach Park, who | 4 | | passed away on October 23, 2015; and
| 5 | | WHEREAS, Donna Moncivaiz was born on October 29, 1961 in | 6 | | Evanston and grew up in Lincolnshire; her parents were Lawrence | 7 | | Hicks and Jeanne (nee Watry) Hicks; she married Jim Moncivaiz | 8 | | on December 21, 1985 in Deerfield; and
| 9 | | WHEREAS, Donna Moncivaiz was employed in the legal field | 10 | | her entire career and was greatly liked and respected by her | 11 | | colleagues; and
| 12 | | WHEREAS, Donna Moncivaiz loved spending time with her | 13 | | family and friends and cheering on the Chicago Blackhawks; she | 14 | | also had a passion for educating others about melanoma and was | 15 | | instrumental in passing legislation that banned minors from | 16 | | using tanning beds in the State of Illinois; and
| 17 | | WHEREAS, Donna Moncivaiz was preceded in death by her | 18 | | mother and her grandson, Logan Biddle; and
| 19 | | WHEREAS, Donna Moncivaiz is survived by her husband, Jim; | 20 | | her daughters, Morgan (Steve) Hogan and Mallory (David) Biddle; |

| 1 | | her grandchildren, Kathryn, Steven, Alexys, Kayleigh, and | 2 | | Lucas; her father Lawrence (Lois) Hicks; her siblings, Claudia | 3 | | (Jeff) Crouch, Elizabeth Hicks, and David Hicks; and her many | 4 | | aunts, uncles, cousins, nieces, nephews, and close friends; | 5 | | therefore, be it
| 6 | | RESOLVED, BY THE SENATE OF THE NINETY-NINTH GENERAL | 7 | | ASSEMBLY OF THE STATE OF ILLINOIS, that we, along with her | 8 | | family and friends, mourn the passing of Donna Jeanne | 9 | | Moncivaiz; and be it further
| 10 | | RESOLVED, That a suitable copy of this resolution be | 11 | | presented to the family of Donna Moncivaiz as an expression of | 12 | | our sympathy.
