AMENDMENT TO HOUSE BILL 1584

AMENDMENT NO. ______. Amend House Bill 1584 on page 3, line 26, by deleting "and"; and

on page 4, by replacing lines 1 through 4 with the following:

"(15) whether the facility is a for-profit or not-for-profit facility; and

(16) whether the facility is or is part of a continuing care retirement community."; and

on page 4, by deleting lines 14 through 24; and

by deleting all of pages 5 through 10; and

on page 11, by deleting lines 1 through 5.
