AMENDMENT TO HOUSE BILL 1261

2    AMENDMENT NO. ______. Amend House Bill 1261, AS AMENDED, by
3replacing everything after the enacting clause with the
4following:
 
5    "Section 5. The Vital Records Act is amended by changing
6Section 5 as follows:
 
7    (410 ILCS 535/5)  (from Ch. 111 1/2, par. 73-5)
8    Sec. 5. (1) The State Registrar of Vital Records shall:
9    (a) Administer and enforce this Act and the rules and
10regulations issued hereunder;
11    (b) Direct, supervise, and issue instructions necessary to
12the efficient administration of a statewide system of vital
13records, the Office of Vital Records, and be custodian of its
14records;
15    (c) Direct, supervise, and control all activities of local
16and subregistrars;

1    (d) Prescribe and distribute such forms as are required by
2this Act and the rules and regulations issued hereunder;
3    (e) Arrange the certificates in a systematic manner;
4prepare and maintain a comprehensive index; permanently
5preserve and keep all such records either in the original or in
6photographic or micro-photographic form;
7    (f) Prepare and publish reports of vital statistics of this
8State, and such other reports as may be required by the
9Department.
10    (g) Prepare and transmit monthly a report to each county
11assessor and each mass transit district indicating the name,
12address, date of birth, date of death, and social security
13number of each decedent who resided within the county or mass
14transit district and died within the reporting period. The
15State Registrar of Vital Records shall adopt rules to implement
16this subsection (g).
17    (2) The State Registrar of Vital Records may delegate such
18functions and duties as are necessary to properly carry out the
19provisions of this Act.
