 1                    AMENDMENT TO SENATE BILL 1073

 2        AMENDMENT NO.     .  Amend Senate Bill 1073 by  replacing
 3    everything after the enacting clause with the following:

 4        "Section  5.  The Illinois Dental Practice Act is amended
 5    by changing Section 8.05 as follows:

 6        (225 ILCS 25/8.05)
 7        (Section scheduled to be repealed on January 1, 2006)
 8        Sec. 8.05. Social Security Number on license application.
 9    In addition to any other information required to be contained
10    in  the  application,  every  application  for  an  original,
11    renewal, or  restored  license  under  this  Act  must  shall
12    include the applicant's Social Security Number.
