 5    on  page  1,  line  20,  by replacing "Act" with "Act and the
 6    principal business office of the physicians' group is located
 7    in a municipality with  a  population  of  less  than  50,000
 8    inhabitants  and  is within 5 miles of the principal business
 9    office of the health care plan".
