| 2 | | WHEREAS, The members of the Illinois Senate are saddened to | 3 | | learn of the death of Bradley J. Lewis DC, DNBHE of Crystal | 4 | | Lake, who passed away on October 24, 2017; and
| 5 | | WHEREAS, Brad Lewis was born in Brown City, Michigan to | 6 | | Jack and Donna Lewis on April 13, 1949, the oldest of three | 7 | | boys; he grew up in Capac and Davison, Michigan, where he made | 8 | | many lifelong friendships; he served in Vietnam from 1970-1971; | 9 | | and
| 10 | | WHEREAS, Brad Lewis attended the National College of | 11 | | Chiropractic in Lombard and earned a Doctor of Chiropractic | 12 | | Degree; he later became a Homeopathic Practitioner; he | 13 | | practiced Chiropractic and Homeopathy in Crystal Lake for 32 | 14 | | years; he was dedicated to helping his patients and counseling | 15 | | those who returned from military service; and
| 16 | | WHEREAS, Brad Lewis was a member of the Illinois | 17 | | Chiropractic Society, the Rotary Club, and the Crystal Lake | 18 | | Chamber of Commerce; he donated to Gift of Life so others could | 19 | | live on; beyond his practice, he had a passion for trap | 20 | | shooting, was an avid reader, and had a knack for crossword | 21 | | puzzles; and

| 3 | | WHEREAS, Brad Lewis is survived by his wife of 47 years, | 4 | | Carol (Hintz) Lewis; his son, Benjamin Lewis; his daughter, | 5 | | Heidi Lewis; his mother; his brothers, Brian (Shirley) and Jack | 6 | | (Rik England) Lewis; and his many nieces and nephews; | 7 | | therefore, be it
| 8 | | RESOLVED, BY THE SENATE OF THE ONE HUNDREDTH GENERAL | 9 | | ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of | 10 | | Bradley J. Lewis DC, DNBHE and extend our sincere condolences | 11 | | to his family, friends, and all who knew and loved him; and be | 12 | | it further
