1 HOUSE RESOLUTION

2     WHEREAS, The members of the Illinois House of
3 Representatives wish to recognize Dr. Sadiq Mohyuddin, Talat
4 Gul Mohyuddin, and Dr. Yusuf Mohyuddin for establishing the
5 Mohyuddin Endowment in support of The Mohyuddin Pulmonary Lab
6 at OSF Saint Anthony's Health Center; and

7     WHEREAS, The Mohyuddin Pulmonary Lab is designed to
8 increase awareness and the prevention of pulmonary diseases and
9 to support technological advancements in respiratory services;
10 and

11     WHEREAS, Dr. Sadiq Mohyuddin has been on the medical staff
12 of OSF Saint Anthony's Health Center for over 44 years; and

13     WHEREAS, Dr. Yusuf Mohyuddin joined Saint Anthony's
14 Physician Group in 2007; and

15     WHEREAS, An inauguration and dedication ceremony will take
16 place at The Mohyuddin Pulmonary Lab at OSF Saint Anthony's
17 Health Center on June 6, 2015; therefore, be it

18     RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE
19 NINETY-NINTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we
20 recognize Dr. Sadiq Mohyuddin, Talat Gul Mohyuddin, and Dr.

1 Yusuf Mohyuddin for establishing the Mohyuddin Endowment in
2 support of The Mohyuddin Pulmonary Lab at OSF Saint Anthony's
3 Health Center; and be it further

4     RESOLVED, That suitable copies of this resolution be
5 presented to Dr. Sadiq Mohyuddin, Talat Gul Mohyuddin, and Dr.
6 Yusuf Mohyuddin as an expression of our esteem and respect.
