SYNOPSIS AS INTRODUCED:

New Act

Creates the Patient Intake Form Military Veteran Inquiry Act. Provides that a health care provider must include on each patient intake form a question asking whether the patient or their spouse has ever served in the military for purposes of tailoring treatment options. Defines "health care provider" and "military veteran".

A BILL FOR

AN ACT concerning health.

Be it enacted by the People of the State of Illinois, represented in the General Assembly:

Section 1. Short title. This Act may be cited as the Patient Intake Form Military Veteran Inquiry Act.

Section 5. Purpose. The purpose of this Act is to improve the tailoring of treatment options for military veterans.

Section 10. Definition. As used in this Act:

"Health care provider" means any physician or public or private facility that provides, on an inpatient or outpatient basis, preventive, diagnostic, therapeutic, convalescent, rehabilitation, mental health, or intellectual disability services, including general or special hospitals, skilled nursing homes, extended care facilities, intermediate care facilities, and mental health centers.

"Military veteran" means a person who has served in any component of the U.S. Armed Forces, including reservists; members of the National Guard of the United States; or the National Guard of any state, the District of Columbia, a commonwealth, or a territory of the United States.

Section 15. Health care providers; patient intake forms. A health care provider must include on each patient intake form a question asking "Have you or your spouse ever served in the military?".
