 1        AN ACT with respect to schools.

 2        Be it enacted by the People of  the  State  of  Illinois,
 3    represented in the General Assembly:

 4        Section 5.  The School Breakfast and Lunch Program Act is
 5    amended by changing Section 5 as follows:

 6        (105 ILCS 125/5) (from Ch. 122, par. 712.5)
 7        Sec.   5.  Application  for  participation  in  programs.
 8    Applications  for  participation  in  the  school   breakfast
 9    program,   the  school  lunch  program,  the  free  breakfast
10    program, and the free lunch program shall be  made  on  forms
11    provided  by  the State Board of Education and filed with the
12    State Board of Education, through the Regional Superintendent
13    of Schools.
