Synopsis As Introduced
Amends the Vital Records Act. Requires the State Registrar to prescribe and distribute a form for a certificate of birth resulting in stillbirth. Requires a person who files a fetal death certificate to also prepare a certificate of birth resulting in stillbirth. Requires that the certificate be filed as in the case of a certificate of live birth and be offered to the parent or parents of the stillborn child. Provides that a parent of a stillborn child who was born before the effective date of this amendatory Act of the 93rd General Assembly may apply for a fetal death certificate. Effective immediately.

Senate Committee Amendment No. 1
Deletes all substantive provisions from the bill. Retains only the short title and effective date.

Senate Floor Amendment No. 3
Amends the Vital Records Act. Requires the State Registrar to prescribe and distribute a form for a certificate of stillbirth. Provides that a person who files a fetal death certificate shall, only upon request by the woman who delivered the stillborn fetus, also prepare a certificate of birth resulting in stillbirth. Requires that the certificate be filed in the same manner as a certificate of live birth. Provides that a parent of a stillborn child who was born before the effective date of this amendatory Act of the 93rd General Assembly may request for a fetal death certificate. Effective immediately.
