AMENDMENT TO HOUSE BILL 4809

2     AMENDMENT NO. ______. Amend House Bill 4809 as follows:
 
3 on page 1, by inserting immediately below line 12 the
4 following:
5     ""Caregiver identification card" means a caregiver photo
6 identification card issued by the Department of Human Services
7 for the purpose of enabling a holder of the card to use a LINK
8 card to obtain food stamp benefits or cash on behalf of a
9 person entitled to benefits funded under the federal Food Stamp
10 Program.
11     "Caregiver" has the same meaning ascribed to that term in
12 Section 12-21 of the Criminal Code of 1961."; and
 
13 on page 1, by inserting immediately below line 13 the
14 following:
15     ""Facility provider" means an individual representing a
16 facility that provides residential services to persons who are

1 developmentally disabled.
2     "Guardian" has the same meaning ascribed to that term in
3 Section 10 of the Health Care Surrogate Act."; and
 
4 on page 2, by inserting immediately below line 1 the following:
5     "(d) Beginning on the effective date of this amendatory Act
6 of the 96th General Assembly, the Department of Human Services
7 shall issue a caregiver identification card to the facility
8 provider, guardian, or caregiver of a person entitled to
9 benefits funded under the federal Food Stamp Program. Every
10 caregiver identification card must include on its face a
11 current photo of the facility provider, guardian, or caregiver
12 of the person entitled to benefits, the name of the person
13 entitled to benefits, and a statement that clearly identifies
14 the card holder as having the legal capacity to use the named
15 person's LINK card. Beginning on the effective date of this
16 amendatory Act of the 96th General Assembly, a facility
17 provider, guardian, or caregiver must present a valid caregiver
18 identification card when using a LINK card to obtain food stamp
19 benefits or cash on behalf of a person entitled to benefits
20 funded under the federal Food Stamp Program.".
