AMENDMENT TO HOUSE BILL 5477

2    AMENDMENT NO. ______. Amend House Bill 5477 by replacing
3everything after the enacting clause with the following:
 
4    "Section 5. The Animal Welfare Act is amended by changing
5Section 7 as follows:
 
6    (225 ILCS 605/7)  (from Ch. 8, par. 307)
7    Sec. 7. (a) Applications for renewal licenses shall be made
8to the Department, shall be in writing on forms prescribed by
9the Department, shall contain such information as will enable
10the Department to determine if the applicant is qualified to
11continue to hold a license and shall be accompanied by the
12required fee, which shall not be returnable.
13    (b) As part of its annual renewal application, an animal
14shelter or animal control facility shall report beginning
15inventory and intake and outcome statistics from the previous
16calendar year. The report of intake and outcome statistics

1shall include the following:
2        (1) The total number of dogs, cats, and other animals,
3    divided into species, taken in by the animal shelter or
4    animal control facility, in the following categories:
5            (a) surrendered by owner;
6            (b) stray;
7            (c) impounded other than stray;
8            (d) confiscated under the Humane Care for Animals
9        Act;
10            (e) transferred from other licensees within
11        Illinois;
12            (f) transferred into or imported from outside
13        Illinois;
14            (g) transferred into or imported from outside the
15        United States;
16            (h) born in the shelter or animal control facility.
17        (2) The total number of dogs, cats, or other animals,
18    divided by species, that leave its facilities, in the
19    following categories:
20            (a) returned to owners;
21            (b) placed or sold;
22            (c) euthanized;
23            (d) euthanized at the owner's request;
24            (e) died in custody;
25            (f) transferred to another licensed animal shelter
26        or animal control facility within the State;

1            (g) transferred to an out-of-state non-profit
2        animal shelter or facility;
3            (h) missing, stolen, or escaped;
4            (i) released in field or put into a program under
5        which animals are trapped, neutered, and released; and
6        (3) The ending inventory or shelter count per species
7    at the end of the last day of the year.

9    Section 99. Effective date. This Act takes effect January
