Section 240.630  Determination of Eligibility

a)         A determination of eligibility is an examination of each applicant's/client's circumstances to determine the need for receipt of Community Care Program (CCP) services.  This determination shall consist of analyzing, evaluating, and documenting, when necessary, current, full and complete information obtained from personal assessment of the applicant/client in the home of said applicant/client (except in the prescreening process).

b)         The assessment shall be accomplished by use of the standard assessment tool (Community Care Program Determination of Need (DON)) promulgated by the Department for purposes of such assessment.

c)         An applicant/client may be denied or terminated at any time that ineligibility is determined based upon any eligibility criteria that are not met, as required by Sections 240.710 through 240.875.
