Section 240.620  Home Visit

a)         Determinations and redeterminations of need for Community Care Program (CCP) services shall be administered during a visit to the home of the applicant/client except: when conducted in the prescreening process or when a CCP client has been hospitalized or placed in any type of institution and will be discharged to the community in less than sixty (60) calendar days.

b)         A home visit shall be conducted, and a redetermination of need administered in the client's home, within fifteen (15) calendar days from the date of a client's discharge from a hospital or other institution.

c)         A home visit shall not be required in the conduct of determinations or redeterminations of need following hospitalization for routine procedures as defined in Section 240.160.
