Section 595.120  Reporting of Accidents or Incidents

a)         A report of each accident or incident shall be filed with the Commission not later than thirty (30) days after such accident or incident occurred.  The report shall be made on Form Ill. C. C. (P.U. 7) Accident or Incident Report, or the appropriate U.S. Department of Transportation Incident Report Form, RSPA F7100.1 (3-84) for Gas Distribution Systems, or RSPA F7100.2 (3-84) for Gas Transmission and Gathering Systems.  If the accident investigation is incomplete after the expiration of the thirty (30) day period, an additional report will be filed upon its completion, or every ninety (90) days until the investigation is completed.

b)         Information required by this form must be provided to comply with Section 6 of the Illinois Gas Pipeline Safety Act.  Failure to do so may result in penalties pursuant to Section 7 of the Illinois Gas Pipeline Safety Act (Ill. Rev. Stat. 1987, ch. 111⅔, par. 557).
